Insurance Requirements
INSURANCE REQUIREMENTS

Contractor shall maintain in force with companies having AM Best rating of A VIII or higher, unless approved in writing in advance by COMPANY:

  1. Commercial General Liability Insurance with a combined single limit of not less than $1,000,000 per occurrence and $2,000,000 aggregate.
  2. Workers' Compensation Insurance to the extent required by applicable laws and employer’s liability coverage of at least $1,000,000 per occurrence.
  3. Comprehensive Automobile Liability Insurance, in the event a motor vehicle is to be used in Contractor's performance of services, covering liability arising out of any auto (owned, hired and non-owned) with a combined single limit of not less than $1,000,000
  4. Insurance of not less than $5,000,000 per occurrence; (the coverage’s specified in (i) through (iv) all on an occurrence basis)
  5. Professional Liability Insurance (extending such coverage to the services contemplated by this Agreement) with a $5,000,000 limit of liability (on a claims made basis)
  6. Employee Dishonesty Insurance, including a Third Party Fidelity endorsement and an endorsement naming Staffing Tree. it’s affiliates and subsidiaries as a loss payee, with a $1,000,000 limit of coverage (on a discovery basis).

All policies shall be on a primary basis and without any right of contribution from any insurance carried by COMPANY. The insurance coverage specified in #1, #3 and #4 above shall name Staffing Tree. its affiliates and subsidiaries and its employees and agents as additional insureds. Contractor shall provide proof of such insurance to COMPANY prior to the commencement of this agreement and at each subsequent policy renewal date. The certificate shall provide for not less than thirty (30) days written notice to COMPANY prior to policy cancellation, non-renewal or material change.
